Each resident’s needs are monitored through an individual assessment evaluation initially and ongoing care plan reviews at least quarterly. A care plan team of interdisciplinary members including our onsite physician medical director and registered dietician coordinate the clinical and therapeutic individual plans to promote independence in a supportive environment. Physical therapy, occupational therapy, speech therapy, and restorative nursing are all part of our inclusive rehabilitation program.
