Progressive health care plans are developed through team efforts which include residents and their families. Each resident’s needs are monitored through an individual assessment evaluation initially and ongoing care plan reviews at least quarterly. A care plan team of interdisciplinary members including our onsite physician medical director and registered dietician coordinate the clinical and therapeutic individual plans to promote independence in a supportive environment. Physical therapy, occupational therapy, speech therapy, and restorative nursing are all part of our inclusive rehabilitation program.
